Who Is Eligible?
Adults aged 55 and older
Individuals who need supportive housing but do not require skilled nursing or medical care
Individuals who can perform basic self-care with light assistance
Individuals who are able to live in a shared community environment respectfully
Referrals from case managers, shelters, hospitals, and community agencies are welcome
Required Documents Checklist
Please have the following documents ready for your intake:

☐ Medical information summary (list of medications, known allergies, primary care physician — for reference only, not for treatment)
☐ Any referral paperwork from your case manager, social worker, or referring agency
